沖縄県の頭頸部癌の罹患率—癌登録からの統計的観察

Incidence of Head and Neck Carcinoma in Okinawa:Statistical Observation from the Prefectural Cancer Registry

　はじめに

　沖縄県では琉球大学医学部の設置以来，耳鼻咽喉科学教室を中心に頭頸部癌の治療を行ってきた。われわれは頭頸部癌の治療のなかで，沖縄県における頭頸部の各癌の罹患頻度が日本の他の地域と異なるであろうとの印象を持っていた。なかでも咽頭癌，特に下咽頭癌の罹患頻度は高率であろうと推察していた。しかし沖縄県全体としての患者数，罹患頻度の把握は1施設では困難であった。沖縄県では1987年から沖縄県衛生環境研究所を中心に癌登録が開始され，頭頸部癌の県全体の状況が把握可能となった。今回1988年から1990年までの3年間の登録資料に基づき，頭頸部癌の各癌の罹患頻度を算出し，統計学的検討を行った。その結果に若干の文献的考察を加えて報告する。

The incidence of head and neck carcinoma in Okinawa was obtained from the prefecture-wide data of Okinawa Prefectural Cancer Registry which started in 1988. In the period of 1988 to 1990, the crude incidence of each carcinoma in male and female was as followed; the tongue 2.3, 1.2, the salivary gland 0.6, 0.3, the oropharynx 2.2, 0.1, the nasopharynx 0.5, 0.1, the hypopharynx 1.8, 0.2, the nasal and paranasal cavity 1.1, 0.5, the larynx 3.3, 0.4, respectively. The incidence of the oropharyn-geal and hypopharyngeal carcinoma in Okinawa was high with statistical significance compared with that of other than Okinawa in Japan.
