初診時頭頸部，上部消化管重複癌症例の検討

Synchronous Double Cancers of the Head and Neck and the Upper Digestive Tract

　はじめに

　頭頸部悪性腫瘍は上部消化管との重複癌の発生頻度が高く，治療法や予後との関連から，内視鏡検査による上部消化管のスクリーニングの重要性が指摘されている^1〜3）。琉球大学耳鼻咽喉科では，1988年より第1外科の協力のもとに頭頸部悪性腫瘍症例に対して，初診時に上部消化管検索としてヨード染色を伴った内視鏡検査を施行している。今回われわれは，初診時の内視鏡検査で上部消化管に同時性重複癌の発見された頭頸部癌について，発生部位，進展度，治療法，予後などについて検討したので報告する。

From 1988 through 1994, 13 patients with synchro-nous double cancers of the head and neck and the upper digestive tract were treated.

The primary site of the head and neck were the hypopharynx in 8 cases, the oropharynx in 4 and the oral cavity with the hypopharynx in 1.

Eleven of 13 cases were operated, but in 8 cases, recurrence or distant metastasis were recognized.

It was considered that control of the head and neck cancers and post operative chemotherapy provided better treatment result.
