Anomaly of the internal jugular vein was found incidentally during left conservative neck dissection in a 56-year-old male patient with neck lymph node metastasis from cancer of the oral cavity. He received laser operation to the primary lesion which recurred in his left neck 7 months later. A conserva-tive neck dissection was performed, and anomaly of the internal jugular vein was found locating at the laryngeal level. The internal jugular vein bifurcated into two branches and then joined together approxi-mately 2 cm above the bifurcation.

There have been several reports of the versatile anomalies of the internal jugular vein, however, we could not find a similar anomaly case in the litera-tures.
