はじめに
平均寿命の延長に伴い,頭頸部悪性腫瘍においても患者の高齢化が認められ1),高齢者の手術症例も増加している。しかしながら高齢者の手術では,全身の予備力の低下,合併症の存在,余命との関係からその手術適応や術中,術後の管理などが問題となることが少なくない。
今回われわれは,高齢者,特に80歳以上の頭頸部悪性腫瘍患者の手術症例について検討したので報告する。
From 1983 to 1992, 24 patients of 80 years of age or above presented with malignant tumors in the head and neck were operated on.
The primary sites included the larynx (8 cases), oral cavity (7 cases), external ear canal, mesophar-ynx (3 cases), hypopharynx (2 cases) and the thy-roid gland (1 case).
The results showed that 13 patients (54%) sur-vived, 7 patients (29%) died of primary disease and 4 patients (17%) died of other disease.
No operative mortality was noted, and it suggest-ed that operative treatment was feasible in the cases even with 80 years of age or above.
