当教室における二重癌についての検討

Multiple Primary Cancer of the Head and Neck

　はじめに

　近年の悪性腫瘍の診断技術の進歩，治療成績の向上ならびに平均寿命の延長に伴い，頭頸部領域の重複癌の報告が増加している。

　しかしながら重複癌の中でも，第一癌が他領域の癌ですでに根治的治療がなされている場合，臨床的にはあまり問題とはならないことが多い。

　今回われわれは，琉球大学耳鼻咽喉科学教室で経験した二重癌症例のうち，同時性に発症した症例ならびに，異時性二重癌のうち第一癌が頭頸部癌の症例について，その発生部位，治療，予後などについて検討したので報告する。

From 1981 to 1992, 16 synchronous double cancers and 15 metachronous double cancers related to head and neck were treated.

The incidence was high in the hypopharynx, and larynx in metachronous double cancer cases.

The second cancers occurred in the esophagus and other various organs in metachronous double cancer cases, although the esophagus and head and neck in synchronous double cancer cases.

During follow up studies after treatment of first head and neck cancers, it was recommended to examine the upper digestive tract and the head and neck because of high risk of occurence of second cancer.
