A 14-year-old girl presented with an enlarged and asymptomatic mass in the right submandibular region without any palpable intraoral mass. MRI demonstrated a well-defined cystic mass located anterior to the right submandibular gland, which was continuous with a sublingual gland. This find-ing strongly suggested a plunging ranula. Under general anesthesia, the plunging ranula was excised with the removal of intraoral sublingual glands along with intraoral evacuation of cystic contents. MRI is a useful tool for preoperative evaluation. The removal of the sublingual glands enables us to avoid a high risk of recurrence, and is the reason-able option for treatment of a plunging ranula.
