当教室における舌癌症例の検討

Review of 50 Patients with Cancer of the Tongue

　はじめに

　舌癌は，頭頸部領域では比較的発生頻度が高く治療する機会も多い腫瘍であるが，早期の腫瘍でも治療後局所再発や頸部リンパ節転移をきたし治療に難渋することも少なくない。今回われわれは，1986〜1996年までに当教室で手術を中心に治療した舌癌症例について治療成績をまとめ，今後の治療成績の改善のため検討を行ったので報告する。

From 1986 through 1996, 50 patients with cancer of the tongue were treated.

The patients with stage I or II of primary lesion were treated by laser, and the patients with advanced locoregional tumor were treated surgi-cally followed by reconstruction.

Prognoses of these cases were as follows : 31 patients (62%) survived, 14 (28%) died of primary disease, and 5 (10%) died of the other diseases. The 5-year survival rate for stage I , II, III, IV were 77%, 59%, 38% and 40% respectively. The overall 5-year survival rate was 58%.

The neck recurrence was developed in 14 patients, local recurrence in 5 and locoregional recurrece in 4. The neck recurrence was controlled with following neck dissection, but the patients with local recur-rence were not salvaged.
