当科における最近の甲状腺手術例の検討（第4報）

A Clinical Study on Thyroid Operations in Our Department

　緒言

　甲状腺は頭頸部領域に位置する臓器であり，この部は一般外科との境界領域である。しかし，甲状腺の疾患では，咽喉頭異常感や嗄声などの症状を主訴に耳鼻咽喉科を受診することが少なくない^1）。また，甲状腺手術は周囲に喉頭，食道，気管といった臓器を有しさらに甲状腺裏面には反回神経も走行していることから，われわれ頭頸部外科医が手術を行うべきと考えている。

　当科では1986年より甲状腺疾患に対して積極的な治療を開始し，過去3回手術例の検討を行い第1〜3報までの報告をしている^1〜3）。今回第4報では，1986年から1995年までの術後の合併症である上皮小体機能低下症や反回神経麻痺について検討した。

Four hundred sixtyeight patients with thyroid diseases were operated on in our department for 9 years. There were 71 males and 397 females.

Postoperative hypoparathyroidism occurred in 107 (70.4%) patients of 152 patients underwent subtotal and total thyroidectomy. In cases of malig-nant tumor, it occured in 95 (82.6%) patients of 115 patients.

Postoperative recurrent nerve palsy occurred in 8 (3.6%) patients of 223 patients in benign tumor and 41 (16.7%) patients of 245 patients in malignant tumor. In cases of malignant tumor, transient palsy occurred in 22 (9.0%) patients and permanent palsy occurred in 19 (7.8%) patients.
