当科における最近の甲状腺手術例の検討（第3報）

A Clinical Study on Thyroid Operation in Our Department

　緒言

　甲状腺手術は甲状腺が周囲臓器とくに喉頭，気管と隣接し，さらに反回神経が裏面を走行することから，一般外科との境界領域疾患とはいえ耳鼻咽喉科頭頸部外科が行う機会がふえている。当科でも1986年より甲状腺疾患に対し積極的に治療を行っており，過去に2回手術例の検討をしている^1,2）。前回第2報^2）では初期の3年間（1986年〜1988年）の報告をしているが，その後さらに4年が経過し手術症例数も飛躍的に増加した。また手術例数のみならず，その内容にも変化がみられる。今回1989年から1992年までの4年間で経験した241例を1986年から1988年までの96例に加え，合わせて報告するとともに，前半の3年間の96例との比較検討を行った。また1988年より術前検査として穿刺吸引細胞診を取り入れていることは第2報^2）で報告したが，1989年からはさらに当院で行われている人間ドックに甲状腺エコーが取り入れられたこと^6）から手術症例数およびその内容が変化しており，これも含めて検討を行った。

Three hundred thirty-seven patients with thyroid diseases were operated on in our department for 7 years.

There were 45 males and 792 females. Pathologi-cal diagnosis was benign tumor in 167 patients, malignant in 179 patients.

Ultrasonography was used for screening of thy-roid tumor in our health care unit since 1989.

Echo-guided aspiration biopsy cytology of the thyroid is an excellent method for eliminating un-necessary operations in patients with thyroid nod-ules.
