当科における最近の甲状腺手術例の検討（第2報）

A Clinical Study on Thyroid Operations in our Department

　緒言

　甲状腺は，頭頸部領域にありながら一般外科との境界領域であるため，現在のところでは一部の施設を除き，耳鼻咽喉科よりも一般外科で取り扱われることの方が多い。しかしながら甲状腺疾患の中には，咽喉頭異常感，嗄声等の耳鼻咽喉科的症状を主訴として，耳鼻咽喉科を受診するものも少なくない^1）ため，耳鼻咽喉科でも積極的に取り組むべき疾患であろうと思われる。

　当科では1986年より，甲状腺疾患に対し積極的に治療を行っているが，山田が当科における甲状腺手術例の検討（第1報）^2）を報告して以来1年7ヵ月が経過し，手術症例数も96例と前回報告時の約3倍に増加した。

Ninety-six patients with thyroid diseases were operated in our department for 3 years. There were 11 males and 85 females.

Pathological diagnosis was follicular adenoma in 32 patients, adenomatous goiter in 17, cyst in 10, Basedow's disease in 4, chronic thyroiditis in 3, adenocarcinoma in 25, anaplastic carcinoma in 2, squamous cell carcinoma in one, clear cell carcinoma in one, other in one.

Ultrasonography has been used routinely since July 1986, and echo-guided aspiration biopsy cytology has been used routinely since April 1988.

Aspiration biopsy cytology is useful in the preoperative diagnosis.
