甲状腺癌二次治療例の検討

Clinical Study on Second Operation of Thyroid Carcinoma

　緒言

　甲状腺癌の大多数を占める分化癌は一般的には極めて良好な経過をたどり，ヒト癌中最悪といわれる甲状腺未分化癌と対極に位置する。実際に，再発をきたしても救済手術が可能なことが多い。

　この癌の生物学的特殊性^1）に加え，比較的若い女性に発生することを考慮し，より侵襲の少ない手術で十分であるとする考え^2,3）があるが，一方で再発を繰り返すうちに生物学的悪性度が変化し，結果として不幸な転帰をたどる症例も少なからず認められるため，一次手術において積極的な拡大治癒手術を求める意見^4,5）もある。

　このように，甲状腺分化癌の一次治療法に関しては未だ施設間で相違があり，一定の方針が確立していないのが現状である。必要最小限で，かつ適切な手術術式は何かを見出すために，当科において経験した再発甲状腺癌に対する二次治療例を検討した。

Among 282 patients operated on for thyroid car-cinoma in our department, We studied 11 patients who had undergone reoperation of thyroid car-cinoma between April 1986 and March 1996.

There were no cases underwent regional neck dissection in the first operation in other depart-ments.

We concluded that unilateral regional neck dissec-tion should be the procedure of choice in patients for intra-hemithyroid differentiated carcinoma without cervical lymph node metastasis.
