頭頸部腺様嚢胞癌症例の検討

Adenoid Cystic Carcinoma of the Head and Neck:Clinical Survey of 30 Cases

　はじめに

　頭頸部腺様嚢胞癌は比較的まれであるが，原発巣や転移などに関して扁平上皮癌とは異なった臨床像を呈することが知られており，診断，治療にあたってはそのことを十分理解しておくことが重要と思われる。そこで1973年より1994年の22年間に当科で治療を行った頭頸部腺様嚢胞癌について検討を行ったので報告する。

Thirty patients with head and neck adenoid cystic carcinoma who were treated between 1973 and 1994, were enrolled in this study. Patients consisted of 14 males and 16 females. Major salivary glands and paranasal sinuses were common site. The overall 5 and 10 year survival rates determined by Kaplan -Meier analysis were 71.9 and 54.3%. Predominance of the solid component on pathological examination might indicate a worse prognosis for 5-year sur-vival. Patients with major salivary gland tumors obtained the best 10-year survival rate (79.9%). While that of those with nasal cavity and paranasal sinus tumors was worse (29.8%). Local control achieved by complete surgical resection of the pri-mary tumor would improve the long-term survival rate.
