同一耳下腺内に同時に2種類の良性腫瘍を認めた1症例

Two Different Benign Tumors in the Parotid Gland:A Case Report

　はじめに

　耳下腺腫瘍は頭頸部腫瘍のうち約2％を占め^1），比較的よくみられる疾患であるが，その大部分は単発性である。多発性の耳下腺腫瘍としてはワルチン腫瘍が最も多く，その他には，良性腫瘍の悪性転化例や転移性の耳下腺腫瘍などが報告されている。しかし，同一耳下腺内に同時に2種類の良性腫瘍が認められたという報告はまれで，そのなかでも良性多形腺腫とワルチン腫瘍が混在した症例は，筆者らが渉猟しえた範囲では20例の報告がみられるのみである。

　今回われわれは，同一耳下腺の浅葉に良性多形腺腫が，深葉にワルチン腫瘍が同時に存在し，耳下腺腫瘍摘出術により両腫瘍とも摘出した症例を経験したので報告する。

We present a case of Warthin's tumor with a simultaneously found pleomorphic adenoma in the same parotid gland. The occurrence of synchronous major salivary gland tumors with different his-tologic types in one patient appears to be unique. Moreover, the combination of different benign tumors is extremely rare. Only 29 cases of coinci-dence of different kinds of benign tumors occurred in one patient were found in our review of the literature. In Japan, only 2 cases were previously reported, and tumors occurred in the parotid gland and histologic type showed Warthin's tumor and pleomorphic adenoma.
