頭頸部における腺様嚢胞癌の検討

Investigations of Adenoid Cystic Carcinoma in Head and Neck

　はじめに

　腺様嚢胞癌は頭頸部領域に好発する癌の1つであり，一般的に臨床経過が長いにもかかわらず治療困難な癌である。以前より，腺様嚢胞癌の病理組織型別による予後の違いが論じられているが^1〜8），今回われわれは当科で診療した腺様嚢胞癌症例について組織分類学的検討を行ったので報告する。

Nineteen cases of adenoid cystic carcinoma in head and neck were studied. There were 8 cases in the nasal cavity and paranasal sinuses, 6 in the major salivary glands, 3 in the external auditorymeatus, 2 in the pharynx.

According to the predominant pattern on his-tologic examination, adenoid cystic carcinoma was classified to two types. Nine tumors showed no solid pattern and 8 tumors solid pattern. The 5-year survival rate was 100 % in no solid pattern, 51 % in the solid pattern. The 10-year survival rate was 86 % in no solid pattern, but 34 % in solid pattern. The prognosis of tumors without solid pattern was significantly better than with solid pattern.
