　はじめに

　手術不能の気管・気管支狭窄による呼吸困難に対し，気管内挿管は有効であるが患者の不快感やquality of life （QOL）の低下は著しいものがある。近年，メタルステントやシリコンステントなど種々の気管用ステントが考案され，使用されるようになってきた。メタルステントはイントロデュサーを用いファイバースコープ下で留置することができ，内科医や胸部外科医でも行える。1990年に発表されたDumon型シリコンステントは，ステントの外側に突起がついており，留置後の移動を防ぐようになっている^1）。なおその留置はDumon型硬性気管支鏡（Dumon型ベンチレーションブロンコスコープ）下で行うため，耳鼻咽喉科医が依頼されることが多い（図1）。

　われわれは，最近3例の気管・気管支狭窄にDumon型シリコンステントを留置したので，その使用経験を報告する。

Three patients with inoperable tracheobronchial stenosis were treated by Dumon silicone tracheo-bronchial stent. Patient 1 was a 56-year-old man with recurrent malignant thymoma. Patient 2 was a 68-year-old man who underwent right sleeve pneumonectomy for lung cancer and patient 3 was a 62-year-old woman with recurrence of undiffer-entiated carcinoma of the thyroid gland. All patients were treated by Dumon stent. Patients 1 and 3 were relieved of dyspnea and discharged from hospital.

Tracheal intubation is effective for dyspnea caused by tracheobronchial stenosis, but it causes intolerable pain and discomfort, and requires the patients complete rest. Dumon stent is recommend-ed for patients with tracheobronchial stenosis, even those in the end-stage, to relieve dyspnea and improve quality of life.
