眼症状を呈した蝶形骨洞真菌症の2症例

Two cases of aspergillosis of the sphenoid sinus

Ⅰ　はじめに

　副鼻腔真菌症は寄生型（非侵襲型）と破壊型（侵襲型）に分類される^1）。寄生型は真菌が副鼻腔内にとどまり周囲組織には浸潤しないため，手術を行って副鼻腔を開放し真菌を除去することで予後良好な経過を示す。一方，破壊型は骨破壊や血管浸潤を伴いながら周囲組織に進展するため，大きく安全域をとることが難しい副鼻腔領域では手術で完全に真菌を除去することが難しく，予後不良な経過をたどることが多い。

　副鼻腔のなかでも蝶形骨洞は，眼窩や視神経，内頸動脈などの重要臓器に隣接している。このため，蝶形骨洞から炎症や圧迫が周囲組織に波及すると複視や視力障害などの視覚器障害が生じやすい。さらに破壊型真菌症が原因の場合は内頸動脈や脳幹を障害し不幸な転機をたどる場合も多い。

　今回われわれは，複視と頭痛という同様の症状で発症し，一方は寄生型もう一方は破壊型と考えられた蝶形骨洞真菌症の2症例を経験したので，若干の文献的考察をふまえ報告する。

　Two patients showed visual symptoms,and the one was diagnosed as having noninvasive type of aspergillosis of the sphenoid sinus. The other was diagnosed as of invasive type. Voriconazole was given to the second case in addition to endoscopic sinus surgery. Although it is known that invasive sinus aspergillosis is often fatal,both patients recovered.

　Voriconazole is effective in aspergillosis and is recommended as the first choice of antifungal agents. It is important to give voriconazole for invasive sinus aspergillosis as soon as possible.
