パッチテスト陰性の慢性中耳炎に対する鼓室形成術

Tympanoplasty in Chronic Otitis Media with Fixation of the Ossicles

　緒言

　パッチテストで聴力改善が得られない慢性中耳炎の多くに耳小骨の固着や離断がみられる。これらに対しては，耳小骨の連鎖形成術が必要であるが，主として自家骨・軟骨や同種骨・軟骨，または人工耳小骨を使ったIII型変法が用いられている。その成績は著者によって異なるがおおむね良好の報告が多い^1〜5）。確かに，キヌタ骨欠損や，キヌタ・アブミ関節の離断がある場合にIII型変法を行うことは合理的であるといえる。しかしながら，耳小骨連鎖周囲が硬い線維性肉芽で充満されていても連鎖そのものは，離断なく保たれている場合もよく経験するところである。このような場合に，連鎖の可動性が不良であるからといって，すぐにII型変動を行うことには疑問がある。Atticotomyを行い，周囲肉芽を十分に除去することによって，I型のままでもかなりの聴力改善を得ることができるからである。そこで，われわれはパッチテスト陰性であった慢性中耳炎症例に対して行った手術方法ならびにその術後成績について報告する。

Tympanoplasty was performed on 44 patients with chronic otitis media, and 21 out of 44 patients received reconstruction of the ossicles, mainly using incus or cortical bone, and 23 patients received no reconstruction of the ossicles (type I tympano-plasty and atticotomy). Fifteen out of the 23 patients had less than 10 dB of hearing improve-ment with patch test before operation. This paper deals whit operative technique for hearing improve ment and hearing result in these 15 patients.
