外耳道に腫瘤を形成し，顔面神経麻痺を発症したMTX-LPDの1例

A case of MTX-LPD associated with a tumor in the ear canal and facial paralysis

はじめに

　メトトレキサート（MTX）は，現在では関節リウマチ（RA）のうち予後不良群に対する第一選択として広く用いられている^1）。MTX関連リンパ増殖性疾患（methotrexate-associated lymphoproliferative disorders：MTX-LPD）は，低用量のMTX投与中の患者に発生するリンパ増殖性疾患であり，実臨床においては悪性腫瘍との鑑別が問題となる。今回，外耳道に腫瘤を形成した稀なMTX-LPDの1例を経験したので，文献的な考察を加えて報告する。

　Methotrexate is an anti-tumor agent that is also used for rheumatoid arthritis and can cause methotrexate-associated lymphoproliferative disorders（MTX-LPD）.

　We encountered a rare case of MTX-LPD associated with a mass in the external auditory canal.

　The patient was a 74-year-old female with a chief complaint of ear discharge accompanied by facial paralysis. Because of her history of MTX use, a final diagnosis of MTX-LPD was rendered. After 6 courses of R-CHOP chemotherapy, her symptoms immediately resolved. To our knowledge, no similar cases have been reported in the international literature.
