仮道への涙管チューブ留置によりcheese wiringを生じた症例

Case of cheese-wiring due to placement of a lacrimal duct tube in a temporary duct

はじめに

　cheese wiringとは，涙管チューブ挿入後に涙点と涙小管の周囲の軟部組織が，チーズをワイヤーで切ったようにチューブによって切断されることを指す^1）。眼角部の疼痛，霧視，鼻汁のほか，流涙をきたすことがあるが，無症状の症例も報告されている。涙管チューブ挿入後にcheese wiringをきたした症例の報告は4例と少なく，いずれも鼻腔内視鏡下で涙管チューブを抜去されている^2,3）。

　今回われわれは，仮道への涙管チューブ留置によるcheese wiringを生じ，同時に流涙が再発した症例に対して内視鏡下涙囊鼻腔吻合術（endoscopic dacryocystorhinostomy：EN-DCR）を施行し，良好な術後経過を得た症例を経験したため，報告する。

　Case report：A 67-year-old woman. She had cheese-wiring and right lachrymation after a lacrimal canalicular tube was placed. She was referred to our department and underwent a right endoscopic dacryocystorhinostomy（EN-DCR）and septoplasty performed by an ophthalmologist. The lacrimal canalicular tube was found to have strayed into the submucosa near the inferior nasal meatus；therefore, it was removed. The lacrimal sac was opened through the nose, and a lacrimal duct tube was inserted into the superior lacrimal puncta and the common nasal meatus. Four months after surgery, the lacrimation improved, and the lacrimal sac remained open.
