咽後膿瘍から降下性壊死性縦隔炎を併発した2症例

Two cases report of descending necrotizing mediastinitis from posterior pharyngeal abscesses

はじめに

　咽後膿瘍は咽頭後壁粘膜と頸椎の間の咽頭後間隙，危険間隙，椎周囲間隙に膿瘍を形成する疾患で，縦隔にまで炎症が波及して縦隔炎・縦隔膿瘍に至ると，死亡率は約40％とされ予後不良である^1）。特にコントロール不良の糖尿病を合併すると易感染状態となり，重症化する傾向にある^2）。近年の傾向として，抗菌薬の発達などにより幼児の咽後膿瘍の件数は減少する一方で，中高年での咽後膿瘍の件数が増加している^3,4）。

　今回われわれは咽後膿瘍から降下性壊死性縦隔炎を併発した2症例を経験したので，治療における反省点も含めて報告する。

　In this case report, we described two cases of descending necrotizing mediastinitis（DNM） resulting from PPA.

Case 1：A 52-year-old woman with obesity was initially treated with thoracic drainage. Her respiratory condition worsened, and she underwent PPA incisional drainage and subsequent mediastinal drainage due to progression from PPA to DNM.

Case 2：A 65-year-old man with diabetes was treated with extraction of the left tonsil, excision of the PPA to drain the pus, and open chest drainage for the DNM from the PPA. Subsequently, an incision and drainage of the neck abscess were performed.

　PPA and DNM necessitate immediate treatment and long-term swallowing rehabilitation.
