 Benign lymphoepithelial cysts of the parotid gland are one of the symptoms of human immunodeficiency virus(HIV)infection. Herein, we report a case of HIV infection detected by the diagnosis of cysts in the bilateral parotid gland.

 A 49-year-old man presented with a six-month history of left sided facial swelling.

 Magnetic resonance imaging(MRI)revealed a 34×28×40 mm cyst in the left parotid gland, an 18×16×20 mm inclusion cyst in the right parotid gland, and cervical lymphadenopathy. The mass on the left side was surgically removed, and histological diagnosis confirmed a benign lymphoepithelial cyst of the parotid gland. We suspected HIV infection, and subsequent HIV screening tests indicated that the patient was HIV-positive.

 Benign lymphoepithelial cysts are important diagnostic indicators of HIV infection.
