鼻腔内過剰逆生歯牙の1例

A case of inverted supernumerary tooth in nasal cavity

はじめに

　過剰歯は小児歯科領域では比較的よく遭遇する歯の発生異常で，渡辺^1）は1.48％，荻田ら^2）は1.67％の発生率であるとしている。歯の萌出方向については，正常な萌出方向をとるものは順生，正常な萌出方向と逆の方向をとるものは逆生とされている。部位では上顎の正中である正中過剰歯が多いが，上顎小臼歯や下顎小臼歯，上顎洞内に存在するものもあり，鼻腔内の発生はきわめて稀である。上顎洞や鼻腔内に転位するのは逆生歯牙に相当し，耳鼻咽喉科の日常診療で遭遇することはほとんどない。

　今回，近医耳鼻咽喉科で鼻腔内腫瘤を指摘され当院に紹介後，鼻腔内過剰逆生歯芽の診断で内視鏡下に摘出した症例を経験したので報告する。

　We report a rare case of an inverted supernumerary tooth in the nasal cavity. A 6-year-old boy with a mass in the right nasal cavity was referred to our institution. Endoscopy and computed tomography revealed that the mass was a tooth-like structure, which was partially covered with granulation tissue, present on the floor of the right nasal cavity. We extirpated the inverted tooth, which was attached to the inferior turbinate, by endoscopic surgery under general anesthesia. It resembled a canine tooth. Clinicians should be aware of the differential diagnoses for intranasal teeth which may include foreign bodies, rhinoliths, and tumors.
