早期肺腺癌治療後に両側進行性難聴を呈し剖検により判明した癌性髄膜症

A case of leptomeningeal carcinomatosis in a patient with bilateral hearing loss after treatment for early stage lung adenocarcinoma

はじめに

　癌性髄膜症は悪性腫瘍が脳軟膜やクモ膜下腔に転移・浸潤する疾患であり，固形癌の約5〜8％に合併するとされている^1）。臨床症状としては聴覚障害などを呈する場合もあり，耳鼻咽喉科へ紹介されることも多い^2〜5）。癌性髄膜症を診断するためには悪性腫瘍の既往や画像検査から鑑別に挙げ，髄液細胞診で診断をつける必要がある。しかし，悪性腫瘍が根治していると考えられた症例に発生する場合や，画像検査で所見が得られない場合もあり，診断に難渋することがある。今回，早期肺腺癌手術の4年後に両側進行性難聴を呈した癌性髄膜症症例を経験したので報告する。

　Leptomeningeal carcinomatosis（LC）is a rare complication associated with malignant tumors. We report a case of LC diagnosed during autopsy in a patient who presented with progressive hearing loss. A 69-year-old man had a past history of early stage lung adenocarcinoma for which he underwent surgery. Despite many neurological symptoms, imaging inspections did not reveal any specific diseases. Eventually, the patient died owing to worsening respiratory condition. During autopsy, LC was diagnosed due to metastasis of lung adenocarcinoma. LC should be considered in patients with progressive bilateral hearing loss re-gardless of the malignant tumor site.
