はじめに

　頸部を構成する骨や軟骨の形態異常によって，頭頸部領域にさまざまな症状を呈する疾患の一群が知られている。

　患者は76歳の男性で，頸部左回旋時の疼痛を主訴に受診した。CT画像で喉頭斜位，左舌骨大角の過長が確認され，症状と併せて舌骨症候群と診断された。過長舌骨大角および甲状軟骨上角が切除された結果，頸部痛は間もなく消退した。結果的に診断と治療が妥当なものであったことが示唆された。

　以上の症例について文献的考察を加えて報告する。

　A seventy-six years old man had tremendous neck pain on turning his neck toward the left. This pain disappeared in a few seconds. Therefore, there was thought to be pressure pain at bifurcation of the left common carotid artery. The long greater horn of hyoid bone was found in CT scan. Moreover, this long greater horn interfered the cervical spine and common carotid artery. The long part of hyoid bone was excised, and his symptoms disappeared completely.
