 A 29-year-old male was seen with chief complaints of foreign body sensation in the hypopharynx, andodynophagia of three years' duration. Indirect laryngoscopy revealed a round bulge on the right side of the posterior pharyngeal wall. X-ray examination revealed the enlarged greater corn of the hyoid bone impinging upon the pharyngeal mucosa.

 A lateral pharyngotomy was performed, and the enlarged, posteriorly twisted greater corn was identified, and resected under the periosteum. Mucosal and periosteal layers showed moderate inflammatory reaction. Postoperative endoscopy confirmed disappearance of the bulge, and the complaints of the patient were improved. It is suggested that acute upper respiratory infection superimposed upon the mechanically irritated area of the bulge, and active movement of the hyoid system turned the infection into chronic sustained irritation of the area. Bibliographical study was also included.
