肥大せる舌骨大角による舌骨症候群の症例

A CASE OF HYOID SYNDROME DUE TO ENLARGED GREATER CORN

　I．はじめに

　下咽頭異常感を主訴とする患者において，明らかな発生原因を発見することが困難な症例は稀ではない。一般に咽喉頭異常感の成因には，局所的，全身的および心理的なものがあげられ^1），他覚的な検査によるも成因が不明であるときは，心身症的な側面が強調されやすい。症状が重症である患者にとっては，たとえ医師の説明に一応の納得が行くにせよ，成因が不明なことには不満が多い。また，成因に関しては，頻度の高いものの他に，稀なものを考慮する必要がある。

　今回筆者らは，3年間下咽頭不快感を訴えながら特に異常を認められず，後に嚥下困難を訴えたため入院精査を行い，舌骨大角の異常な肥大が，不快感の起原であったと思われる症例を経験したので，ここに報告する。

　A 29-year-old male was seen with chief complaints of foreign body sensation in the hypopharynx, andodynophagia of three years' duration. Indirect laryngoscopy revealed a round bulge on the right side of the posterior pharyngeal wall. X-ray examination revealed the enlarged greater corn of the hyoid bone impinging upon the pharyngeal mucosa.

　A lateral pharyngotomy was performed, and the enlarged, posteriorly twisted greater corn was identified, and resected under the periosteum. Mucosal and periosteal layers showed moderate inflammatory reaction. Postoperative endoscopy confirmed disappearance of the bulge, and the complaints of the patient were improved. It is suggested that acute upper respiratory infection superimposed upon the mechanically irritated area of the bulge, and active movement of the hyoid system turned the infection into chronic sustained irritation of the area. Bibliographical study was also included.
