はじめに

　茎状突起過長症は，変形した茎状突起により舌咽神経や頸動脈周囲の交感神経叢が過度の刺激を受け，その結果，頭頸部や顔面に種々の不快な症状が出現する病態をいう。茎状突起過長症に関するポイントは，①正確な診断，②安全な手術手技，③症状の消失もしくは緩和の達成に集約される。診断については画像検査の発達によりほぼ正確な診断が可能になったが，症状と画像所見が乖離している場合もあるので注意を要する。また安全で有効な手術法を選択することはいうまでもないが，治療が症状の緩和に必ずしも繋がらないケースもみられるため，治療前の入念な説明が不可欠である。以上の点を考慮しつつ自験例を供覧する。

　It is known that many uncomfortable symptoms appear on the face, head and neck in elongated styloid process. It is difficult to diagnose if we do not have enough knowledge about this disease. The patient is a 58-year-old female with symptoms of globus pharyngeus and pressure of the neck. Therefore, she had head, neck and face pain with rotation of her neck. In our investigation, an elongated styloid process was found by panorama X-Ray and CT, 3D-CT. Bilateral styloid processes were removed via oral cavity. After operation, her all symptoms were disappeared quickly and dramatically.
