はじめに

　Castleman病は胸腺腫に類似した組織像を示す縦隔に発症するリンパ節の過形成病変として，1954年にCastlemanらによって最初に報告された疾患である。本症の好発部位は縦隔であり，頭頸部領域での発生は比較的稀である。治療法としては，腫瘤の全摘出が第一選択であり，全身症状を有する症例に対してはこれまで副腎皮質ステロイドの投与が行われてきた。さらに，近年は抗IL-6受容体抗体による治療が行われるようになり，多くの症例で症状が改善したと報告されている。

　われわれは顎下腺に発症した単発性Castleman病の1例を経験した。本症例の概要を文献的な考察を加えて報告する。

　Castleman disease commonly occurs in the mediastinum and is relatively rare in the head and neck region.

　A 36-year-old female complained of a mass in the right submandibular region. This mass was removed by surgical resection with the submandibular gland. After surgery, the mass was diagnosed as unicentric Castleman disease. There was no evidence of recurrence in the patient 6 months after the operation.
