顎二腹筋後腹の浅層を走行する外頸動脈の1例

Anatomical variation of the external carotid artery coursing through the superficial layer of the posterior belly of the digastric muscle: A case report

はじめに

　顎二腹筋は，頸部郭清術をはじめとする多くの頭頸部外科手術においてきわめて重要なメルクマールとなる構造物である。顎二腹筋の浅層には顔面静脈が走行することが多いが，それ以外の内・外頸動脈，内頸静脈などの主要血管や，副神経・舌下神経・迷走神経などの脳神経といった組織は，通常，同筋の深層を走行することが知られている^1）。例えばLevel Ⅱa〔American Academy of Otolaryngology-Head and Neck Surgery（AAO-HNS）の頸部リンパ節Level分類〕の郭清上縁操作において，顎二腹筋後腹より浅層の組織は，通常，重要臓器を含まないため，電気メスやエナジーデバイスで比較的スピーディに切離を進めることが可能である。

　今回，下咽頭癌の頸部リンパ節転移に対する頸部郭清術において，顎二腹筋後腹の浅層を外頸動脈が走行している1例を経験した。顎二腹筋後腹の浅層の操作においても，このような走行異常の可能性も念頭に置いて操作を行う必要がある。

　It is commonly known that the main trunk vessels run deep into the posterior belly of the digastric muscle. We report a case of the external carotid artery running through the superficial layer of the posterior belly of the digastric muscle. Intraoperatively, we identified the posterior belly of the digastric muscle and observed a pulsating blood vessel running through the superficial layer of the muscle. We confirmed that this vessel was the external carotid artery because the facial artery diverged and traveled into the deep part of the digastric muscle. Surgeons should be mindful of these anatomical variations in blood vessels.
