後縦隔Castleman病の1例

A case of posterior mediastinal castleman disease（hyaline vascular type）

Castleman病はリンパ増殖性疾患の一つで，組織学的に硝子血管型（hyaline vascular type）と形質細胞型（plasma cell type），またその混合型（mixed type）がある。硝子血管型が80～90％と多く，単発のリンパ節腫大を呈する。形質細胞型は10～20％でみられ，単発のこともあるが，多中心性Castleman病（multicentric Castleman disease）として多発リンパ節腫大を示す。

27-year-old man was referred to our hospital because of incidentaly detected posterior mediastinal mass. On CT, the mass appeared smoothly with a well-defined border and was enhanced persistently. On MRI, the mass was hyperintense on the T2-weighted images and isointense to skeletal muscle on the T1-weighted images. It was hyperintense on diffusion weighted images and had low ADC value. laparoscopic operation was performed and the histological diagnosis was castleman disease（hyaline vascular type）.
