頸部Castleman病の1例

A Case of Castleman's Disease in the Neck

　はじめに

　Castleman病は反応性のリンパ節腫大を主徴とするリンパ増殖性疾患である。縦隔に好発し，耳鼻咽喉科領域には比較的稀な疾患であり，リンパ節腫大が限局している限局型と，全身性に多発性リンパ節腫大を伴う全身型に分けられる^1）。成因については炎症説，腫瘍説，過誤腫説など諸説があり意見の統一をみていない^2）。

　今回われわれは，頸部に発生したCastleman病の1例を経験したので報告する。

A case of Castleman's disease in the right side of the neck in a 16-year-old female patient was report-ed.

We performed neck 3 D-CT scan and MRI in this patient. The mass was hypervascular, but not adher-ed to blood vessels. Operation was performed and histopathological diagnosis was the hyaline-vascu-lar type of Castleman's disease. There was no evi-dence of recurrence after operation. It is very def-ficult to diagnose Castleman's disease preoperat-ively. We must take this disease into consideration when we encounter head and neck masses.
