内視鏡下に摘出した鼻中隔神経鞘腫の1症例

A case of nasal septal schwannoma

Ⅰ．はじめに

　鼻副鼻腔腫瘍のなかでも，鼻中隔原発腫瘍は稀である^1）。過去に報告された鼻中隔腫瘍の病理診断では良性腫瘍・悪性腫瘍を併せて40種類を越える多様な組織型が報告されており，その鑑別診断は多岐にわたる^1）。そのなかで神経鞘腫は比較的稀な組織型で過去に国内7例，海外13例の報告をみるのみである^2～8）。今回われわれは偶然発見された鼻中隔神経鞘腫の1例に対し，鼻内内視鏡手術を行う機会を得たので，その臨床経過を若干の文献的考察を加えて報告する。

　We present a rare case of nasal septal schwannoma. Only 13 cases had been reported in English literature and 7 cases in Japanese. The patient is a 73-year-old female who was pointed out a mass on radiological exermination of the skull coincidentally. Computed tomography scan and magnetic resonance imaging showed a spherical mass arising from the nasal septum. The mass was removed completely with endonasal endoscopic sinus surgery. Recently some cases of endoscopic resection were reported and our case was also well adapted to endoscopic sinus surgery because of small amount of bleeding and clear margin.
