上顎に発生した悪性エナメル上皮腫の1症例

A case of malignant ameloblastoma of maxilla

Ⅰ．はじめに

　エナメル上皮腫は良性の歯原性腫瘍のなかでは，比較的頻度が高く，主に下顎に発生する^1）。病理組織学的に良性腫瘍とされているが，局所的には骨破壊性に発育し，十分な安全領域を設けた手術の施行にもかかわらず，再発傾向が高い。稀に悪性化し，悪性エナメル上皮腫と呼ばれる。この悪性エナメル上皮腫も主に下顎に発生し，上顎に生じた報告例は少ない^1）。

　今回われわれは上顎に発生し，悪性転化したエナメル上皮腫を経験したので，症例を呈示して若干の文献的考察を加えて報告する。

□Abstract

　We experienced a case of recurrent malignant ameloblastoma of the maxilla with an 18-year interval after resection in a 70-year-old man suffered from a swelling in the right gingival area with maxillary invasion. Denker's operation was performed and then the patient discharged without radiotherapy. This patient was treated for a benign ameloblastoma in the same region in 1977, therefore he showed rare malignant change of ameloblastoma with a long-term interval in 1987.

　We discussed malignant change of ameloblastoma in our case and reviewed the reported cases.
