内視鏡下に摘出しえた鼻中隔多形腺腫の1例

Case of nasal septal pleomorphic adenoma resected by endoscopic nasal surgery

はじめに

　多形腺腫は耳下腺や顎下腺などの大唾液腺や，口蓋や口唇などにある小唾液腺に生じる。小唾液腺は口腔内と咽頭の至る所にみられるが，加えて同様の腺組織が，とりわけ，鼻腔，副鼻腔，喉頭の粘膜にみられる^1）。なかでも鼻腺は咽頭，喉頭，気管などの小腺と同様に，口腔内小唾液腺と類似の腺組織と考えられており^2,3），唾液腺にみられる腫瘍と同様の組織像を呈する腫瘍，すなわち多形腺腫が発生する腺の1つとされている。特に鼻腔内の鼻腺は，鼻中隔に最も多く，以前から鼻中隔の多形腺腫は稀な腫瘍ではあるが数多くの報告がなされている。また多形腺腫は，その腫瘍の性質上，十分なマージンを付けて切除することを要求される。今回，われわれは，内視鏡下に摘出しえた鼻中隔原発の多形腺腫を経験したので，文献的考察を加えて，ここに報告する。

　Here, we report a case of nasal septal pleomorphic adenoma. A 50-year-old male complained of swelling around the nose. Endoscopic findings revealed a nasal septal tumor in the right nasal cavity. A preoperative biopsy revealed a pleomorphic adenoma. An endonasal endoscopic en bloc resection was performed and achieved with negative surgical margins. Intranasal pleomorphic adenoma is thought to have a cellular pattern with few areas of stromal predominance, which accounts for the lower incidence of recurrence. The surgical approach is considered important, and the treatment outcomes should be closely monitored.
