鼻中隔粘膜より発生し内視鏡下に摘出しえた多形腺腫の1例

A case of pleomorphic adenoma that occurred from the nasal septal mucosa and was removed endoscopically

はじめに

　多形腺腫は唾液腺組織由来の良性上皮性腫瘍であり，全身の筋上皮細胞の存在する腺組織に発生する可能性がある。頭頸部領域での好発部位は大唾液腺であるが，稀に鼻腔，口蓋，咽喉頭，気管などに発生する^1）。鼻副鼻腔原発の多形腺腫の報告は稀である。治療の第一選択は手術加療だが，悪性転化をきたすことがあるためサージカルマージンをとって腫瘍を一塊にして摘出することが重要である。術式としては鼻内法と鼻外法があるが，術式にガイドラインはなく安全に腫瘍を摘出できる方法を選択する必要がある。今回われわれは鼻中隔粘膜より発生し内視鏡下に摘出できた多形腺腫の症例を経験したので，若干の文献的考察を加え報告する。

　Pleomorphic adenomas are rarely occurring benign epithelial tumors found in the nasal cavity. As pleomorphic adenoma can undergo malignant transformation, it is essential to remove the tumor en bloc with surgical margins. We present a case in which the tumor originated from the nasal septal mucosa and was removed endoscopically. This patient was a 37-year-old man who developed a pleomorphic adenoma in his nose. We removed the tumor en bloc with surgical margins. Endoscopic removal may be feasible if the entire tumor and its base can be confirmed.
