内視鏡下に摘出した鼻腔神経鞘腫の1症例

A case of nasal neurinoma extirpated by endoscopic surgery

Ⅰ．はじめに

　鼻・副鼻腔には良性・悪性を含めさまざまな腫瘍が発生し，ときには診断や治療に苦慮することがある。そのなかで神経鞘腫は良性腫瘍ではあるが，病理組織診断でも確定診断がつかず複数回の生検が行われることがある。また，摘出術を行う際にも審美的な面や機能障害を防ぐうえで，慎重に術式を選択する必要がある。今回，われわれは生検で鼻ポリープと診断し，内視鏡下で摘出術を施行した後に，摘出標本の病理検査にて神経鞘腫との確定診断を得た症例を経験した。本症例の手術術式および方針について，文献的考察を加えて報告する。

　Cases of neurinoma in the nasal and paranasal sinus were often reported,and there are various considerations about how to extract it. We removed a tumor in nasal cavity by endoscopic surgery. At first this tumor was diagnosed as nasal polyp,but it turned out to be nasal neurinoma after the operation. Neurinoma is hard to diagnose before surgery both macroscopically and pathologically,so we should always keep its possibility in mind.
