Nanophthalmosに伴うuveal effusionのインドシアニングリーン螢光眼底造影所見

Indocyanine green angiographic features of nanophthalmos with uveal effusion

（展示141）　真性小眼球症nanophthalmosに伴うuveal effusionの2症例を経験し，強膜開窓手術前および手術後にインドシアニングリーン螢光眼底造影（ICG造影）を行った。ICG造影では，造影早期から著しい脈絡膜血管外漏出がみられ，脈絡膜血管の描出は不明瞭であった。造影晩期には脈絡膜組織内にびまん性過螢光をみ，脈絡膜血管の著明な透過性亢進が存在することがICG造影により証明された。これらは，脈絡膜血流の強いうっ滞と眼外への脈絡膜組織液の通過障害によると考えられた。本症例では，手術により網膜剥離は治癒したが，ICG造影所見は手術前後で変化はなく，脈絡膜組織内にsubclinicalに強い組織液のうっ滞が残っていることが推定された。

Uveal effusion developed as complication of nanophthalmos in two females aged 32 and 44 years each. Retinal detachment subsided following scleral fenestration surgery. Indocyanine green (ICG) angiography showed intense extravasation from the early angiographic phase on. The major choroidal vessels were poorly outlined. Diffuse hyperfluo-rescence developed in the late phase due to dye pooling in the choroid, suggesting the presence of intense stagnation in the choroidal vascular system. Above features persisted even after surgery. These findings suggest that abnormal choroidal dye leakage is subclinically present in uveal effusion in nanophthalmos because of intense congestion of choroidal vessels and impeded trasscleral outflow.
