内因性真菌性眼内炎の病期分類の提案

Proposed classification of stages of endogenous fungal endophthalmitis

　自験例20例40眼および文献的考察を基に内因性真菌性眼内炎の病期分類を行った。従来いわれていた典型的な後極部の円形白色病変が出現する前に，前房内および硝子体中に微細な炎症性細胞の遊出がみられることがあり，これをⅠ期として分類することが早期発見にも大変有用と思われた。また硝子体中に限局性の混濁がみられる時期をⅢ期とし，これをaおよびbに分けたが，これは手術時期を考えるうえでこの時期が重要と考えたためである。

　したがって本症は，Ⅰ期：前房内と硝子体中に炎症性細胞の遊出する時期

　Ⅱ期：後極部に円形白色病変をみる時期

　Ⅲ期a：上記に加えて硝子体中に軽度の限局性の混濁をみる時期

　Ⅲ期b：限局性の硝子体混濁が中等度以上になる時期

　Ⅳ期：上記に加えて網膜剥離がある，または高度の硝子体混濁のため眼底が透見できない時期と分類するのがよいと考えた。

A classification of endogenous fungal endoph-thalmitis is proposed based on a review of affected 40 eyes, 20 cases. Stage 1: appearance of inflamma-tory cells in the anterior chamber and the vitreous. Stage 2: appearance of white round lesions in the posterior fundus. Stage 3a: appearance of mild opacity in the vitreous. Stage 3b: moderate or severe opacity in the vitreous. Stage 4: retinal detachment or totally opaque vitreous.
