　眼内レンズ（IOL）挿入術後の細菌性眼内炎6例の臨床経過を報告した。水晶体嚢外摘出術およびIOL挿入術術後の症例が5例，超音波水晶体吸引術術後の症例が1例であった。6例中起炎菌が検出されたのは2例で，そのうち1例の前房水からStaphylococcus epidermidisが，1例より硝子体切除術の硝子体サンプルから　Sta—phylococcus aureusが培養された。発症時期が術後4日から7日で比較的急性の経過をとった3例のうち早期に硝子体切除術を行った2例は，視力予後良好であった。晩期に硝子体切除術を行った1例は視力予後不良であった。発症時期が術後10日から18日で比較的亜急性の経過をとった3例のうち弱毒菌が検出された症例では晩期に硝子体切除術を行ったが，視力予後良好であった。術後の炎症を無菌性眼内炎と考えステロイドを投与した症例では，視力予後が不良であった。IOL挿入術後の眼内炎では，臨床症状に加え発症時期をも考慮し，治療方針を選択すべきであると考えられた。

We observed 6 eyes of endophthalmitis after intraocular lens (IOL) implantation. The surgical procedure consisted of extracapsular cataract extraction with IOL implantation in 5 eyes and phacoemulsification-aspiration with IOL implanta-tion in 1. Staphylococcus epidermidis was isolated from the aqueous in one eye and Staphylococcus aureus from the vitreous in another. Visual out-come of acute endophthalmitis was favorable in 2 out of 3 eyes following early vitrectomy. Visual outcome was poor in one eye following vitrectomy 38 days after onset of acute endophthalmitis.Favor-able visual outcome resulted in one eye following vitrectomy 4 months after onset of subacute endo-phthalmitis due to S. epidermidis. Endophthalmitis after IOL implantation ought to be treated accord-ing to the time of its onset and clinical features.
