ぶどう膜炎症例に対する眼内レンズ挿入術

Intraocular lens implantation in eyes with uveitis

　ぶどう膜炎が鎮静してから4年以上を経過した60歳以上の4例6眼に水晶体嚢外摘出術と後房レンズ挿入術を行い，全例，0.5以上の術後視力が得られた。術後早期には66.7％に一過性の眼圧上昇やフィブリンの出現がみられたが，重篤な炎症の再燃はみられなかった。症例によっては，ぶどう膜炎の既往があっても眼内レンズ挿入術の適応となると考えられるが，炎症が長期間鎮静していた症例を選択するなど，症例の選択，手術時期の決定には慎重さが必要である。

We performed extracapsular cataract extraction with posterior chamber lens implantation in 6 eyes, 4 patients, with uveitis. All the cases were 60 years of age or elder. All the eyes were free of active inflammation for at least 4 years before surgery. A final visual acuity of 0.5 or better was achieved in all the eyes. There was no recurrence of uveitis after surgery except temporary elevation of intraocular pressure in 4 eyes and fibrin in the anterior chamber in 4 eyes during the postoperative one week. Cata-ract surgery with intraocular lens implantation is a safe procedure with careful selection of patients and after sufficient longterm control of uveitis.
