片眼の視神経腫脹を伴った両眼急性網膜壊死症候群の1症例

A case of bilateral acute retinal necrosis syndrome with unilateral swelling of optic nerve

　眼底の後極部付近の滲出性病変で発症し，切除硝子体液の水痘・帯状疱疹ウイルス抗体価が高値を示し，これが原因ウイルスと考えられた両眼性の急性網膜壊死症候群の51歳女性に，CTおよびMRIにて片眼の視神経の腫脹を認めた。治療として，アシクロビルおよびγ-グロブリンの点滴静注，ステロイドの内服，汎網膜光凝固術に加え，片眼の網膜剥離の予防を目的として硝子体切除術と網膜輪状締結術を行ったが，術後に網膜前増殖膜による牽引性網膜剥離が発生した。本症ではCTやMRIで視神経の状態を検索することも重要であろう。

We diagnosed a 51-year-old female as bilateralacute retinal necrosis syndrome. Computed tomo-graphy and magnetic resonance imaging showedunilateral swelling of the optic nerve. There was anelevation of antibody titer for varicellazoster virusin the specimen obtained by vitrectomy. The swol-len optic nerve showed the same intensity as in thecontralateral one in T₂-weighted MRI images.
