YAGレーザーによる前部硝子体破砕が奏効した有水晶体眼の悪性緑内障

Nd: YAG laser disruption of anterior vitreous was effective for phakic malignant glaucoma

　原発閉塞隅角緑内障の手術後に生じた有水晶体眼の悪性緑内障に対し，YAGレーザーによる前部硝子体破砕が奏効した2例を経験した。

　1例はトラベクレクトミー術後，他例はトラベクロトミー後の縫合不全例で，2例とも軽度の小眼球と小角膜があった。悪性緑内障と診断し，保存的治療を行ったが，瞳孔領と虹彩切除部から水晶体を通し，YAGレーザーによる前部硝子体破砕を行い，悪性緑内障は寛解した。YAGレーザーによる悪性緑内障の治療は，無水晶体眼や偽水晶体眼だけでなく，有水晶体眼にも有効で，観血的手術を行う前に試みる価値のある，新しい治療法であることが示された。

Malignant glaucoma developed in 2 eyes withprimary angle closure glaucoma followingtrabeculectomy or trabeculotomy. We deliveredNd:YAG laser to the anterior vitreous facethrough the pupil and the iris coloboma. After twosessions with about 80 bursts each, a lasting curewas achieved in both eyes by relieving the ciliaryblock and reestablishment of deep anterior cham-ber.
