手持ち式眼圧計Tono-Pen Ⅱの使用経験

Clinical evaluation of hand-held pneumatic tonometer Tono-Pen Ⅱ

　手持ち式圧平式眼圧計Tono-Pen Ⅱが開発されたのでこの眼圧計を試用した。205眼を対象とし，眼圧はGoldmann圧平式眼圧計とTono-Pen Ⅱで測定し比較した。平均眼圧はGoldmann眼圧計では16.9±5.1mmHg，Tono-Pen Ⅱでは16.8±6.1mmHg，相関係数は0.85であった。Goldmann眼圧計では21mmHg以上にもかかわらずTono-Pen Ⅱでは20mmHg以下と測定された偽陰性は2.4％，偽陽性は4.4％で平均誤差は2.3mmHgであった。Tono-Pen Ⅱは小型軽量であり，しかも測定体位を選ばない。暗室では使用しにくいという欠点はあるが測定値の平均がデジタル表示され検者間の差が起こりにくく，その測定値も信頼しうると思われた。

　We evaluated the feasibility of a handheld applanation tonometer Tono-Pen Ⅱ developed by Bio-Rad. Tonometric readings were compared withthose by Goldmann applanation tonometer. Measurements in 205 eyes gave 16.9±5.1 mmHg forGoldmann tonometer and 16.8±6.8 mmHg for Tono-Pen Ⅱ. There were good correlations between thetwo readings (r=0.85). The incidence for falsenegative and false positives was 2.4% and 4.4%respectively. The mean error was 2.27 mmHg.

　Tono-Pen Ⅱ proved thus to be reasonably accurate for clinical use. As minor drawbacks, it israther difficult to use in darkroom and is expensive. It is lightweight, small, and can be used to patientsin any body position.
