眼内レンズの偏心が静的視野へ及ぼす影響

Effect of decentration of intraocular lens on static visual field

　眼内レンズの偏心が視野にどのように影響するかを検討した。緑内障，角膜疾患，眼底疾患のない偽水晶体眼で，矯正視力が0.5以上の4例を対象とし，ハンフリー自動視野計で中心視野30°を測定した。眼内レンズの偏心が著明な症例では，周辺視野に感度の低下があり，瞳孔領にpositioning holeやレンズ辺縁の露出していた症例では，全域に感度の低下がみられた。原因として，グレアーの他に偏心による非点収差や湾曲収差などが考えられた。眼内レンズの挿入された緑内障眼の静的視野の評価には，レンズの偏心による視野の障害に注意を要する。

We studied the visual field within 30 degrees from the fixating point by an automated perimeter by Humphrey. Visual field was normal in a non-glaucomatous eye with the intraocular lens in cor-rect position. Various degrees of depression of sensitivity were present in the visual field in 3 eyes with decentration or baring of the edge of the intraocular lens. Glare or aberration seemed to be the cause of the impaired visual field in these 3 eyes. The present findings seemed to offer guide-lines in the interpretation of visual field in pseudo-phakic glaucomatous eyes.
