網膜中心静脈閉塞症をきたした硬膜頸動脈海綿静脈洞瘻の1症例

A case of central retinal vein occlusion secondary to dural carotid cavernous fistula

　経動脈塞栓術により，眼症状が改善したにもかかわらず，4か月後に網膜中心静脈閉塞症（CRVO）が発症した硬膜頸動脈海綿静脈洞瘻（硬膜CCF）の1例を経験したので報告した。本症例では硬膜CCFの治癒過程の中でCRVOが発生したものと考えられるが，塞栓術後に存続した軽度高眼圧が網膜内循環に悪影響を与えた可能性も否定できない。硬膜CCF症例においては塞栓術後もCRVOの発症を念頭において詳細な眼底検査の施行とともに，眼圧のコントロールを厳密に行う必要があると思われた。

A 57-year-old female presented with diplopia ofone month's duration. The left eye showed dilatedconjunctival vessels and abducent palsy. Cerebralangiography showed ipsilateral dural carotid cav-ernous fistula (CCF). Transarterial embolizationresulted improvement of clinical picture. The leftvisual acuity decreased to 0.1 5 months later. Weobserved central retinal vein occlusion in theaffected eye. Dural CCF was in a healed state oncerebral angiography.

We attribute central retinal vein occlusion in thispatient to stasis in intraretinal circulation in addi-tion to presumed thrombosis in the central retinalvein after rgression of dural CCF. Elevation ofintraocular pressure seemed to have been anothercontributing factor.
