We observed 3 cases of central retinal vein occlusion (CRVO) following spontaneous carotid cavernous fistula (SCCF). One case, an 82-year-old female, underwent spontaneous resolution after an episode of vitreoretinal hemorrhage due to CRVO. Another case, a 67-year-old female, developed impending and progressive CRVO. After embolization therapy for SCCF, the CRVO further progressed and resulted in final visual acuity of 0.06. The third case, a 58-year-old female, developed initial signs of CRVO after embolization. The CRVO resolved soon without visual impairment. The findings show that the CRVO was due to formation of thrombus in the central retinal vein and that it may progress, paradoxically, during the process of attempted cure of SCCF.
