先天縮瞳の1例

A case of congenital miosis

　先天縮瞳症はいかなる散瞳薬にも反応しない直径1mm位の小瞳孔と，虹彩紋理不明瞭を特徴とするまれな先天性疾患であるが，小瞳孔のため周辺部眼底の詳細な記載はみられない。今回の症例は75歳男性で生来瞳孔径1mm弱で，種々の散瞳薬でも全く反応せず，虹彩紋理は不明瞭で収縮襞は少数認められたが，捲縮輪，放射状皺襞，小窩はみられなかった。同症例に水晶体嚢外摘出術と後房レンズ挿入手術をし，術後眼底検査では黄斑耳側から周辺部にかけて強い網膜萎縮がみられ，同部位の視野欠損もみられた。手術時得られた虹彩の組織検査では瞳孔散大筋，括約筋は欠如しており，遺伝形式は単独発生例で外胚葉起源の発生異常と推測した。

A 75-year-male sought medical advice for bilat-eral failing vision. Besides advanced cataract, he manifested small and fixed pupil of 1 mm in diame-ter in both eyes. The pupils failed to react to topical mydriatics. During consecutive cataract surgery, we performed large superior iridectomy to prevent possible pupillary occlusion. Histopathological studies of the excisediris showed total absence of dilator muscles.
