網膜中心静脈血栓症をきたした頸動脈海綿静脈洞瘻の4例—自然治癒との関係について

Four Cases of Carotid-Cavernous Fistula with Spontaneous Healing

緒言

　頸動脈海綿静脈洞瘻（Carotid-Cavernous Fis—tula，以下CCFと略）では拍動性眼球突出などの特異な眼症状が現われることが古くから知られているが，眼底に網膜中心静脈血栓症様の大量の出血をきたしたという報告は少ない。また症状は一般に進行性で，自然治癒は稀といわれている。今回著者らは本症の経過観察中に網膜中心静脈血栓症様の眼底出血を伴つて眼球突出などの眼症状がいつたん増強し，その後眼症状が消失して自然治癒をきたしたと思われる症例4例を経験したので報告する。そのうち1例は脳血管造影によつてCCFの自然治癒が証明されたものである。

Four cases, middle aged women, with spont-aneous carotid-cavernous fistula were reported. One patient had bilateral fistulae and in the other remaining cases the fistula was unilateral.

In all cases, spontaneous healing took place gradually after retinal hemorrhages simulating central retinal vein thrombosis appeared. Ocular manifestations of the two cases disappeared soon after carotid angiography. Spontaneous closure of the fistula was demonstrated by carotid angiography in one case.

It was suggested that the spontaneous healing occurred probably due to spontaneous thrombo-sis of the cavernous sinus.
