Manual compression of the common carotid, or Matas procedure, is widely performed in the treatment of spontaneous carotid cavernous sinus fistula. We observed five cases out of a consecutive series of ten treated by this procedure who developed central retinal vein occlusion, CRVO. Ischemic CRVO developed one year after the procedure in two cases. Nonischemic CRVO developed during repeated procedures in three cases. The final visual outcome was poor in four out of five cases. We advocate due precaution in planning Matas procedure by keeping the possibility of consequent CRVO in mind.
