眼内水晶体落下例の水晶体摘出

Removal of posteriorly dislocated lens

　外傷性水晶体脱臼の水晶体全摘術中に水晶体落下を起こした79歳の症例について，水晶体の摘出を行った。

　落下水晶体の摘出術は術中偶発症や合併症が生じやすいため安全な方法が選択されるべきであり，本症例は硝子体手術と開放創からの娩出の併用で行った。

　本方法の手順においては，眼内での水晶体の移動が自在に行われることが必要であり，今回は静水圧を利用してのフルートニードルによる吸着法をとった。

　フルートニードルによる吸着法は，フルートハンドルの小孔を開閉することで水晶体の着脱が容易であり，吸着圧も灌流瓶の高低をかえることにより調節される。静水圧による吸着力は，娩出時の水晶体保持として十分な力を有する。

　落下水晶体の摘出術において，フルートニードルによる水晶体の吸着移動は有用な手段であった。

A 75-year-old male patient had the dislocated firm lens on the retina and the rhegmatogenous retinal detachment after the complicated intracap-sular cataract extraction. We removed the lens by vitrectomy technique and limbal extraction. During the procedure, we used a 19 gauge flute needle to grasp the lens by suction and position near theanterior chamber prior to removal through a lim-bus incision. There was no operative complication. We believe that the lens transposition by the flute needle is an effective method for a management of the posteriorly dislocated lens that requires removal.
