難治性周辺部角膜潰瘍に対するプロテアーゼインヒビターOvomacroglobulinの点眼治療効果

Effect of topical chicken egg white ovomacroglobulin, a protease inhibitor, in severe peripheral corneal ulcers

　プロテアーゼ抑制剤であるニワトリ卵白ovomacroglobulinは，血漿α₂　macroglobulinと相同タンパク質で，プロテアーゼに対して幅広い抑制効果を有している。この1％および0.1％溶液の点眼で難治性の周辺部角膜潰瘍の5症例を，単独1日6回点眼または1％アトロピン点眼との併用で治療した。症例は慢性関節リウマチに合併する周辺部角膜潰瘍2例，蚕食性角膜潰瘍1例，カタル性角膜潰瘍2例である。いずれも点眼後7日以内に潰瘍は消失し，残存する炎症は，低濃度のステロイドホルモンおよび非ステロイド性消炎薬の点眼のみで完治した。潰瘍の再発が1例にあった。潰瘍の修復に先行して自覚症状の改善傾向が認められた。副作用はなかった。

Five cases of severe peripheral corneal ulcers including 2 cases of complicated chronic rheumatoid arthritis, 1 case of Mooren's ulcer and 2cases of catarrhal ulcers were treated by topical application 6 times/day with chicken egg white ovomacroglobulin (ovoM) which possesses a wide inhibitory spectrum to many types of proteases. The therapeutic effect was prominent. All the ulcers were cured within 7 days. There was no side effect.

Subjective symptoms improved before onset of objective improvements of ulcers.
